Community Health Worker/Casemanager

The CHW/Case Manager at Family Promise of the Great Rivers provides comprehensive, hands-on support to families experiencing homelessness or at risk of homelessness in Monroe County. Using a rapid rehousing and housing-first approach, this role helps families quickly transition into stable, permanent housing while also supporting long-term success through employment assistance, financial coaching, health goal setting, and connection to community resources.

The position leads intake and assessment, maintains detailed case documentation in Wisconsin’s HMIS (Clarity), implements the Pathways Community HUB model, and collaborates with volunteers, partner agencies, and landlords to coordinate services. Responsibilities include conducting regular family meetings and home visits, advocating for families, providing crisis support, and helping secure housing, employment, childcare, education, and healthcare resources.

This full-time role requires strong organization, initiative, compassion, and the ability to manage a caseload of 20-30 families in a fast-paced environment. Success is measured by outcomes such as achieving and maintaining permanent housing, reducing shelter stays, and improving family stability. Training and professional development are provided, with opportunities for wage increases based on certification and performance.

Job Description

Job Overview The Family Promise CHW/ CM is responsible for providing comprehensive case management for families experiencing homelessness or imminent homelessness in Monroe County. This organized, compassionate, and motivated person will quickly transition families from homelessness to permanent housing, following a rapid rehousing, housing-first approach. The CHW/CM will also assist families in obtaining outcomes such as, but not limited to, securing employment, increasing earning potential, securing/maintaining housing, and securing a trade/certificate. The CHW/CM will assist families with setting health goals and work with them to accomplish those goals. This is a rewarding but challenging job! Responsibilities for CHW/CM •Lead the intake process for all potential families: conduct intake interviews, talk with referral source(s)/partners, and conduct orientation for new families moving into one of our programs. •Maintain the client's comprehensive case record through detailed documentation within Clarity (Wisconsin's HMIS) and update relevant information in Wisconsin's coordinated entry. •Implement the Pathways Community HUB model to achieve better health outcomes for participants. Document all Pathways Community HUB model implementation and work with all participants in Great Rivers HUB's care coordination systems (CCS) documentation software. •Communicate with Host Coordinators and the Executive Director to ensure hosting congregations have necessary family information (work schedule, food allergies, etc.) Communicate with volunteers to arrange accommodations for families such as, but not limited to, transportation, special needs, unexpected changes in scheduling, etc. Communicate with volunteers when they have concerns or reason that a family is not adhering to shelter policies such as but not limited to being respectful of all parties present, keeping a drug and alcohol-free environment, keeping all medications secured in a lock box provided by Family Promise, having no weapons on-premises, and not having visitors on shelter premises unless approved by Family Promise Staff and Host Coordinators. •Assist with job applications and interview preparation; budgeting/financial coaching; getting families connected to necessary resources (daycare, counseling, educational programs, etc.) •Find housing for families by liaising with landlords and getting families ready as needed: debt repair, ensuring families can get utilities turned on in their name, will have furniture, etc. •Work with other agencies to deliver resources, referrals, and workshops on employment, financial literacy and budgeting, housing plans, educational planning, and healthcare as needed. •Develop relationships with partner organizations and maintain a database of resources for families. •Serve as an advocate for families with community service agencies. •Provide crisis support to families needed. •Conduct weekly individual meetings with network families and monthly meetings with graduates. •Conduct home visits and communicate in a mutually convenient way. •Coordinate volunteers and donations specific to family needs. This could range anywhere from seeking furniture or clothing donations to coordinating volunteers to help with move-in. •Attend monthly case manager virtual meetings with other Family Promise Regional case managers (1 hour/month) •Attend monthly Community of Practice meetings with Great Rivers HUB CHWs in Monroe and surrounding counties. •Attend weekly 45-minute employee meetings to coordinate a weekly plan at Family Promise. •Attend monthly meetings with The Monroe County Homeless Coalition to discuss resources within the area and advocate for filling gaps within those resources. •Additional responsibilities as needed. Qualifications for Case Manager •The successful candidate is enthusiastic, can display good judgment, takes initiative, and helps families achieve outcomes. Must have a can-do, figure-it-out attitude. •Must be detail-oriented, execute projects in a fast-paced, often pressured environment, able to prioritize, self-manage, work independently, and always use good judgment. •Comfortable working with various constituents: guests we serve, faith community volunteers, corporate and foundation partners, etc. •Must be proficient in computer programs and demonstrate a record of tracking outcomes and recording case notes. •Must possess a positive attitude and model this for the families we serve. •The CHW/CM will work 40 hours a week, Monday through Friday, 8:00 a.m. to 4:30 p.m., but may be required to adjust hours based on the families currently being served. Each family we serve has a different set of needs, and finding a creative way to adjust and fill those needs may result in different hours from week to week. e.g., home visits after 4:30 p.m. or on the weekend. •Must pass a background check •Willingness to complete CHW Core Competency training through Great Rivers HUB's training program within 6 months of hire. Work Environment & Managing Success: •The CHW/CM reports to the Shelter Manager/Program Supervisor of Family Promise of the Great Rivers. •Must complete and continue to satisfy all requirements to maintain the status of a CHW (Community Health Worker) as required by Great Rivers United Way •Must complete and continue to satisfy all requirements to maintain licensing to access and utilize Wisconsin's HMIS (Clarity) and utilize the coordinated entry system. •Occasional lifting of materials up to 25 pounds is necessary. •Family Promise is comprised of a small staff and a large pool of volunteers. •The families' outcomes measure this position's success: rate to permanent housing (80%), the rate to retain housing (80%), the length of time in the shelter (less than 45 days), and maintaining a caseload of about 15-20 families or no more than 30 individuals in CCS. •Training will be provided and paid for by Family Promise and it's Partners •Wages will increase upon training completion and then again annually based on job performance. •Gas mileage will be reimbursed from the home office for any work-required destination.
